Inside the knee

What does the meniscus do?

Each knee has two menisci between the thigh bone and shin bone. They help distribute load across the joint and contribute to knee stability.

A meniscus can tear during a twisting injury or gradually as the tissue changes with age.

Symptoms

Common symptoms of a meniscus tear

Read about ACL and meniscus tears
  1. 01

    Joint-line pain

Pain is often felt along the inner or outer side of the knee.

  2. 02

    Swelling or stiffness

The knee may swell after an injury or activity.

  3. 03

    Catching

Some tears cause clicking or catching during movement.

  4. 04

    Locking

A displaced tear can occasionally prevent the knee from straightening normally.

  5. 05

    Pain with twisting or squatting

Turning, deep bending or loaded knee movement may reproduce symptoms.

Assessment

The MRI is only one part of the decision

Meniscus tears are assessed using the history, knee examination and imaging when needed.

An MRI can show the location and pattern of a tear, but the treatment decision should also consider symptoms, knee stability, age of the tear, associated injuries and the condition of the rest of the knee.

Treatment

Many meniscus tears do not need immediate surgery

Non-surgical treatment

When the knee is stable and symptoms are manageable, treatment may include activity modification, rehabilitation and medicines where appropriate.

Arthroscopic treatment

Surgery may be considered when symptoms continue, the knee has significant mechanical symptoms, or the tear pattern is suitable for repair.

The aim is to preserve useful meniscal tissue whenever possible.

If surgery is needed

Meniscus repair or partial meniscectomy

  • Arthroscopic view of a meniscus repair

    Meniscus repair

Where a tear has suitable healing potential, the torn tissue may be sutured or fixed so the meniscus can heal while preserving as much tissue as possible.

  • Arthroscopic view after partial meniscectomy

    Partial meniscectomy

When a damaged unstable portion cannot reasonably be repaired, the surgeon may trim only the tissue that is causing the problem while preserving the remaining meniscus.

Recovery

Repair and trimming have different rehabilitation plans

Recovery after meniscus treatment is based on what was done during surgery.

A repaired meniscus usually needs more protection while the tissue heals. Rehabilitation after limited trimming may progress differently.

Walking, knee movement, strengthening and return to activity should follow the instructions for the specific operation.

Meniscus tear treatment: common questions

No, not every meniscus tear needs surgery. Many meniscus tears can initially be managed without surgery, particularly when the knee is stable and there is no significant locking or persistent mechanical problem.

Meniscus repair aims to preserve and heal the torn tissue. Partial meniscectomy removes only damaged unstable tissue when repair is not suitable.

MRI is often useful for showing the location and pattern of a meniscus tear, but the treatment decision also uses the symptoms and physical examination.

A knee that cannot straighten normally after an injury should be assessed promptly because a displaced meniscus tear or another mechanical problem can block movement.

Yes, most meniscus repair and partial meniscectomy procedures are performed using knee arthroscopy through small incisions.

No, recovery is not the same after meniscus repair and trimming. A repaired meniscus generally needs more protection while the tissue heals. Rehabilitation after limited trimming can follow a different progression.
